I have been practicing as a nurse practitioner since 2012. I am board certified as a Family Nurse Practitioner as well as a Psychiatric Mental Health Nurse Practitioner. The cost of treatment varies depending on the type of service provided and your insurance coverage. We offer competitive self-pay rates and are currently accepting Blue Cross/Blue Shield, United Healthcare, and Aetna. Please contact us to learn more about our fees and payment options.

Refill Protocols
Some Pharmacies automatically fax refill requests that could be out of date. Due to periodic changes in medications, please alert your pharmacy that our office DOES NOT ACCEPT AUTO-FAX REQUESTS FROM PHARMACIES. Our providers have the ability to E-prescribe. Please make sure you have your medications filled at the time of your session with your provider.
To provide the best quality of care all patients are expected to visit their prescriber every three months, at a minimum, for medication management. You may discuss with your prescriber if being seen every six months, either for financial, logistics or other reasons, is more appropriate.
If you miss or cancel your 6-month appointment with your prescriber and you require a refill for your non-controlled medications, you will be charged $10 Physician refill fee for every 30-day refill period past the 6-month mark.
(PLEASE BE AWARE NO CONTROLLED SUBSTANCES WILL BE REFILLED PAST THE 6 MONTH MARK BY STATE AND FEDERAL LAW)
Refill Requests
PHONE: To initiate a refill for your current medication, patients or pharmacies are required to call our prescription refill/after-hours hotline directly at (630) 300-3892. Please have the name of your medication, dose, directions and pharmacy or mail order number ready to follow the verbal prompts. Allow 48 hours turn-around time for all medication refills.

Completion of FMLA, Disability, or other paperwork paperwork for New Patients requires a minimum of (3) three dates of service with your provider. Two (2) dates of service are required to establish an understanding of your situation and provide medication/therapy solutions. The third 3rd date of service is used to fill out paperwork and is needed to ensure that the paperwork is properly filled out and to avoid multiple phone calls to get necessary information to complete paperwork. This appointment is not used as a medication management or therapy visit and is just utilized to fill paperwork.
